Before Your First Appointment

You can save time at check-in by completing your paperwork ahead of time. Our online forms page includes new patient packets in English and Spanish for:

  • All insurance and Medicaid
  • Medicare
  • Auto accident injuries
  • Letter of protection (LOP) and auto LOP cases
  • Workers’ compensation

You may print and bring your completed forms, fax them, or drop them off prior to your appointment.

What to Bring

Please bring a photo ID, your insurance card, a list of your current medications and allergies, any X-rays or records related to your foot or ankle problem, and the shoes you wear most often. If you already use orthotics or braces, bring those as well.

What to Expect During Your Visit

Dr. Bell will review your medical history, ask about your symptoms, and perform a thorough examination of your feet and ankles. Depending on your condition, imaging may be recommended. You will leave with a clear explanation of your diagnosis and a personalized treatment plan, which may include conservative care such as custom orthotics, advanced options like shockwave therapy or ToeFX Light Therapy, or a discussion of surgical options when appropriate.

Do I need a referral to see a podiatrist?

Many insurance plans do not require a referral, but some HMO plans do. Our team can help you check your plan when you call to schedule.

Are forms available in Spanish?

Yes. Every new patient packet is available in both English and Spanish.
